What
is it?
The Mesothelioma Care Passport is a pack designed to help you keep up-to-date with the planned care that you have discussed and agreed with your healthcare professionals.
We hope that it will help you to keep track and make sense of what is happening. It is intended to be a concise record of your care to date, enabling you to understand and discuss with anyone you choose, for example, your GP, clinical nurse specialist, physiotherapist, community nurse, consultant, emergency doctor, clinical trial advisor or anyone else in whom you wish to confide.
If you have requested access to your clinic letters, you can store them here, along with your scan and x-ray reports. It may also be useful as a place to keep your most up to date prescription and medicines. There is also a section for you to store any relevant written information that has been provided to you.
A list of this available information is located at the back of the pack. Please ask your nurse or health care worker if there is information you feel may benefit you or see the downloadable resources section of the website.
The Care Plan section is for you to use as you wish. You may prefer to ask your nurse or health care professional to write a summary of your recent discussion/consultation, including what the next steps are for you, or you may like to make a note of any questions you have for your next appointment.
